Quick answer
If your EOB shows more than expected patient responsibility, check whether your deductible was not yet met, whether coinsurance applies, whether non-covered services are included, or whether the provider was out-of-network.
Check Your Deductible Status
If you have not yet met your annual deductible, the allowed amount goes toward your deductible first. This means you pay more out of pocket for services received early in the year.
Look for Non-Covered Services
Some services may not be covered by your plan. These are billed to you at the full rate and add to your total patient responsibility.
Check Network Status
If the provider was out-of-network, the allowed amount may be based on a lower percentage of the provider charges. You may also owe the difference between the provider charge and the allowed amount.
Frequently asked questions
Why did my copay not cover the entire visit?
Copays are fixed amounts for specific services. If your visit involved additional services like lab work, those have their own cost-sharing beyond the copay.
Can I set up a payment plan?
Yes. Most providers offer payment plans. Contact the billing department to set one up.
